If you are a parent, physician, early interventionist, or case manager wanting to refer a child for speech or feeding therapy, you can fill out a referral form using this link. Before an evaluation can be scheduled, we will need a prescription for services from a physician or dentist. Please note, we currently have a waitlist for therapy services.
